If you are sure that your Internet browser and Adobe Reader are configured as close as possible to the requirements described under 'Browser Requirements' and 'Adobe Acrobat Settings' then you may want to run the Compatibility Test. This will open a Test PDF Form in a new window. If you do not see the phrase, "The Compatibility Test was successful." on the test form, then your system configuration does not meet our requirements. You can still use our application to complete your questionnaire. Once you have finished answering all the questions, the application will ask you for your email address. We can generate your forms and email them to you in the next business day.
Your other option is to save your data to a floppy disk or other media and use a different computer where the Acrobat configuration works successfully.
